Fertility Care and Infertility Evaluation

Fertility care focuses on understanding and managing factors that may affect a couple's ability to conceive. A structured fertility evaluation can help identify possible causes and guide appropriate treatment.

Fertility problems can affect either partner or sometimes involve a combination of factors. Evaluation is therefore planned according to the individual couple's history, symptoms and previous investigations.

When Should You Seek Fertility Evaluation?

A fertility consultation may be appropriate when pregnancy has not occurred despite regular attempts, or earlier when there are known factors that may affect fertility.

Previous pelvic surgery, endometriosis, irregular periods, certain hormonal conditions or known reproductive problems may require earlier assessment.

Common Female Factors Affecting Fertility

Female fertility can be affected by several factors, including:

  • Ovulation problems
  • Irregular menstrual cycles
  • Polycystic ovary syndrome (PCOS)
  • Endometriosis
  • Uterine fibroids
  • Problems affecting the fallopian tubes
  • Problems within the uterine cavity
  • Age-related changes in fertility
  • Other hormonal or reproductive conditions

Fertility Evaluation

Fertility assessment is based on the couple's history and clinical evaluation. Depending on the situation, the doctor may recommend:

  • Detailed reproductive and medical history
  • Menstrual and ovulation assessment
  • Hormonal investigations when indicated
  • Pelvic ultrasound
  • Assessment of the uterus and ovaries
  • Assessment of the fallopian tubes when appropriate
  • Evaluation for conditions such as endometriosis or fibroids
  • Male partner evaluation when required

Fertility and Ovulation

Regular ovulation is an important part of natural conception. Irregular or absent ovulation can make conception more difficult.

If an ovulation problem is identified, treatment is directed at the underlying cause and may include lifestyle measures, medicines or other fertility treatments depending on the individual situation.

Fertility and Endometriosis

Endometriosis can sometimes affect fertility. It may be associated with pelvic inflammation, adhesions or changes affecting the ovaries and fallopian tubes.

Women with symptoms suggestive of endometriosis may require specific evaluation and treatment planning.

Uterine and Tubal Factors

Problems affecting the uterus or fallopian tubes can sometimes interfere with conception. Ultrasound, hysteroscopy, laparoscopy or other investigations may be considered when clinically appropriate.

Fertility Treatment

Treatment depends on the cause identified during evaluation. Depending on the situation, management may include lifestyle changes, treatment of hormonal or reproductive conditions, medicines to support ovulation, surgical treatment of selected conditions or referral for assisted reproductive techniques when appropriate.

The recommended approach is individualised rather than being the same for every couple.

Preparing for a Fertility Consultation

Bring details of previous pregnancies, menstrual cycles, previous fertility investigations, surgeries and relevant medical conditions to your consultation. Previous reports and scans can also help the doctor understand your history.
